Fees and Insurance

INSURANCE:
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

      • Do I have mental health insurance benefits?
      • What is my deductible and has it been met?
      • How many sessions per year does my health insurance cover?
      • What is the coverage amount per therapy session?
        • Is approval required from my primary care physician?

Fees for Self Pay:
Initial Intake Assessment - $200.00
Individual Therapy - $125/ 55 minute session
Individual Therapy - $60/ 30 minute session

Family Therapy - $150/ up to 1 hour

Group Therapy - $75/ up to  1 hour

Week-end Individual - $150/ 55 minute session

Week-end Group/Family - $200/ up to 1 hour

Coaching - $100/ 50 minute session

Consultation - $100/ up to 1 hour

Training - TBD

 

For those whose insurance considers LifeBridge, Inc. an out-of-network provider,  a form will be provided containing information needed to file directly with your insurance company.  

Payment:
All fees and co-pays are payable at the time of your appointment.  Cash, check or credit card is accepted for payment.

Cancellation Policy:
If you do not show up for your scheduled therapy appointment, and you have not given at least 24 hours notice in advance, you may be required to pay a $75.00 fee.
